To receive clinical services, patients must provide accurate, complete medical history and current health information. Withholding relevant health details may affect the quality of care provided. Certain services may require prior diagnostic evaluation or physician referral before proceeding with treatment.
Virtual consultations are conducted by licensed practitioners for non-emergency medical concerns. Telehealth is not a substitute for emergency care — in case of a medical emergency, call 911 or visit the nearest emergency room. Patients must ensure a private, well-lit environment and stable internet connection for the duration of the consultation.
All prescriptions are issued at the sole discretion of licensed healthcare providers based on clinical assessment. We do not guarantee issuance of any specific medication. Controlled substances, if prescribed, are subject to additional regulatory requirements, including periodic in-person reviews and adherence monitoring per applicable law.
Thriving Wellness Source services, including telehealth and urgent care, are not designed for medical emergencies. If you experience symptoms such as chest pain, difficulty breathing, severe bleeding, loss of consciousness, stroke symptoms, or any life-threatening condition, seek immediate emergency medical assistance through your local emergency services.
While our healthcare providers deliver evidence-based care, individual treatment outcomes vary based on patient-specific factors including age, overall health, adherence to treatment protocols, and underlying conditions. No guarantees regarding specific medical outcomes are made or implied by Thriving Wellness Source or its practitioners.
Appointments may be cancelled or rescheduled free of charge up to 4 hours before the scheduled time. Late cancellations or no-shows may incur a nominal fee of $25. Repeated no-shows may result in scheduling restrictions. Emergency situations are always handled with understanding and flexibility upon communication.
Health package subscriptions are billed annually and grant access to the included services for the subscription period. Refunds for unused portions are processed on a pro-rata basis within the first 30 days. Subscriptions auto-renew unless cancelled through your patient portal at least 14 days before the renewal date.
Diagnostic tests are conducted at certified laboratories. Some tests require fasting or specific preparation — failure to follow pre-test instructions may require rescheduling. Lab results are typically available within 24–72 hours and are reviewed by your treating physician before release through your patient portal.
Patients are responsible for providing truthful health information, following prescribed treatment plans, attending scheduled appointments, informing providers of any changes in medications or health status, and treating all healthcare staff with respect. Disruptive or abusive behavior may result in termination of services.
Mental health services including therapy, counseling, and psychiatric consultations are provided by licensed professionals. In cases involving risk of self-harm, harm to others, or acute psychiatric crisis, our providers are mandated by law to take protective action, which may include contacting emergency services or designated emergency contacts.
Patients have the right to access their complete medical records through the patient portal. Requests for record amendments must be submitted in writing and will be reviewed within 30 days. Third-party record releases require signed patient authorization and may incur a reasonable administrative processing fee.
Thriving Wellness Source reserves the right to modify, suspend, or discontinue any service, package, or feature with reasonable notice to active patients. Material changes to these terms will be communicated via email and posted in your patient portal at least 30 days before taking effect. Continued use of services constitutes acceptance of updated terms.
